With the patient lying supine on a firm surface:
- Place your left hand under the patient’s lumbar spine
- Passively flex both hips and knees as far as possible (the normal lumbar lordosis should disappear).
- Keeping the non-test hip flexed, ask the patient to extend the test leg.
- The lumber lordosis should remain absent
- An inability to fully extend the test leg indicates a fixed flexion deformity of the hip on that side.
